Bone resorption is the process by which mineralized bone matrix is broken down and removed. Osteoclasts, a specialized lineage of cells, carry out resorption by secreting acid and proteolytic enzymes that dissolve mineral crystals and degrade the organic bone matrix. This activity is balanced by osteoblasts, which form new bone to replace the resorbed tissue.

Regulation of bone resorption involves cellular signaling pathways and hormonal controls. The RANKL-RANK-OPG axis is central: RANKL stimulates osteoclast formation and activity, while osteoprotegerin (OPG) acts as a decoy receptor to limit RANKL signaling. Other factors include parathyroid hormone, vitamin D, calcitonin, inflammatory cytokines, and mechanical loading. Dysregulation can lead to excessive resorption, contributing to conditions such as osteoporosis, hyperparathyroidism, and certain cancers, or to reduced resorption, with impaired remodeling.

Clinically, resorption is assessed through imaging and, when relevant, histology. Management focuses on treating underlying causes and, in some cases, employing antiresorptive therapies (for example, bisphosphonates or denosumab) to modulate osteoclast activity. If benresorption refers to a specific benign process, its diagnosis and implications should be guided by the tissue context and accompanying clinical findings.
