Wheezen

Wheezen is not a formal medical diagnosis but is commonly used to describe the sound of wheezing during breathing. A wheeze is a high-pitched, musical whistling produced by air flowing through narrowed or obstructed airways. Wheezes are most often heard during expiration, though they can occur on inspiration in some conditions. The term may appear in clinical notes or patient reports, but it should be linked to an underlying cause.

Wheezing can result from conditions that narrow the airways or cause airway inflammation, such as asthma, chronic obstructive pulmonary disease (COPD), viral bronchiolitis, bronchitis, and allergic reactions. It may also be seen with heart failure with pulmonary edema, foreign body aspiration, or certain infections. Less common causes include airway edema, tumors, or sleep-related breathing disorders. Distinguishing the cause typically relies on history, exam, and targeted tests rather than the sound alone.

Diagnosis involves clinical assessment and may include lung function testing (spirometry or peak flow) and imaging if indicated. Differentiating wheeze from other breath sounds, such as crackles or stridor, is important. Management targets the underlying condition: bronchodilators for obstructive diseases, anti-inflammatory therapy when appropriate, and oxygen as needed. In severe allergic reactions, prompt epinephrine is essential, and foreign body obstruction may require urgent removal. People with persistent wheeze should seek medical evaluation to establish diagnosis and develop long-term treatment plans.
