Recementing

Recementing, in dentistry, refers to the process of returning a previously cemented dental restoration—most commonly a crown, but also inlays, onlays, veneers, or bridges—back onto a prepared tooth using luting cement after the original cement has failed or the restoration has become dislodged.

Indications for recementing include dislodgement or loosening of a crown or other indirect restoration, the need to re-seat a restoration after temporary removal, and situations where the fit and margins are still acceptable and no significant damage or decay has occurred. Contraindications include recurrent decay under the restoration, fracture or severe wear of the tooth or restoration, insufficient tooth structure to retain the restoration, or poor periodontal health.

The procedure generally begins with confirming the cause of loosening and evaluating tooth and restoration integrity. Old cement is carefully removed from both the tooth and the interior of the restoration, which is then cleaned and inspected for marginal integrity. A new luting cement is selected based on the restoration material and clinical goals; common options include glass ionomer, resin-modified glass ionomer, zinc phosphate, and resin cements used with adhesive bonding. Isolation with a rubber dam or other technique is often employed to ensure dryness for adhesive cements. The restoration is re-seated, excess cement is cleaned, and the bite is checked and adjusted as needed.

Materials and techniques vary with restoration type and material; resin-based cements with bonding agents are favored for ceramic crowns, while traditional luting cements may be used for metal crowns. Postoperative care focuses on bite adjustments and monitoring for sensitivity or signs of recurrent problems.

Prognosis after recementing depends on tooth structure, marginal integrity, and absence of decay or fracture; successful recementation can restore function and comfort when properly performed. See also dental crown, luting cement, bonding.
