The MDRD formula, or Modification of Diet in Renal Disease formula, is a widely used equation to estimate glomerular filtration rate (GFR). GFR is a measure of kidney function that indicates how well your kidneys are filtering waste products from your blood. The MDRD formula was developed based on data from patients with chronic kidney disease.

The formula takes into account several factors, including serum creatinine levels, age, sex, and race. Initially, it was developed using creatinine measurements standardized to a specific laboratory method. However, since the advent of standardized creatinine assays, a "simplified" or "updated" MDRD formula is now commonly employed. This simplified version is the one most often used in clinical practice today.

The MDRD formula provides an estimated GFR (eGFR) value, typically reported in milliliters per minute per 1.73 square meters of body surface area (mL/min/1.73 m²). This estimated value is crucial for diagnosing and staging chronic kidney disease, monitoring disease progression, and guiding treatment decisions, such as medication dosing and referral to a nephrologist.

While the MDRD formula is valuable, it's important to note its limitations. It can be less accurate in certain populations, including individuals with very high or very low muscle mass, unusual diets, or certain medical conditions. Therefore, other GFR estimation equations, such as the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) equation, are also used and are often preferred due to their improved accuracy across a broader range of patient characteristics. Despite the availability of alternative formulas, the MDRD equation remains a significant tool in the assessment of kidney function.
