Indications and selection: In acute ischemic stroke, IVtPAhoito is considered for patients within the approved time window after symptom onset, commonly up to 4.5 hours. Selection requires brain imaging to exclude intracranial hemorrhage, assessment of cardiovascular status, and evaluation of bleeding risk. It may be used in combination with mechanical thrombectomy in eligible patients, depending on local guidelines and patient factors.

Administration and dosing: The standard dose of alteplase is 0.9 mg per kilogram of body weight, with a maximum of 90 mg. Ten percent is given as an intravenous bolus over 1 minute, and the remaining 90 percent is infused over 60 minutes. Blood pressure and other safety parameters are closely monitored during and after administration, with targets that vary by guideline but generally require careful control of hypertension before and during therapy.

Contraindications and safety: IVtPAhoito is contraindicated in the presence of intracranial hemorrhage on imaging, suspicion of subarachnoid hemorrhage, recent intracranial surgery or stroke, active major bleeding, or uncontrolled severe hypertension.
