Aktivitetsmål

Aktivitetsmål is a term used in Swedish health, social care, and education to describe a clearly defined target for a person’s level of activity or participation. They are used to guide planning, intervention, and evaluation of rehabilitation, learning, and everyday functioning. The aim is to improve the person’s ability to take part in meaningful activities and daily life tasks, not only to treat symptoms or impairments.

Typically, aktivitetsmål are formulated together with the person receiving services, possibly with family or professionals, as part of a care or education plan. They should be relevant to the person’s goals and context, and they link to practical outcomes such as mobility, independence, social participation, or academic performance.

A key feature is that they are specific, measurable, achievable, relevant, and time-bound (SMART). This makes progress observable and accountability clearer. Examples include: “Walk 500 meters unaided in the neighbourhood three times per week within eight weeks,” or “Attend school five days a week and participate in at least one group activity per week for the next two months.”

Progress is monitored and reviewed regularly. When goals are met, planners may set new, more advanced aktivitetsmål; if not, barriers are identified and adjustments are made, such as additional support or changes in the plan. The concept supports person-centered care and outcome-oriented practice.
