Viscosupplementation refers to intra-articular injections of hyaluronic acid or related viscoelastic agents used to treat symptomatic osteoarthritis, most commonly of the knee. The products differ in molecular weight, concentration, and whether the hyaluronic acid is linear or cross-linked. They may be derived from avian sources or produced biotechnologically, with some formulations designed to be more viscous or longer lasting.

Mechanism and administration: Viscosupplements aim to restore the viscoelastic properties of synovial fluid, improving lubrication and shock absorption, and may exert anti-inflammatory effects. Clinical benefit varies and is generally modest, with effectiveness influenced by patient selection. Injections are typically given as a series over several weeks (for example, one injection per week for 3 to 5 weeks), though some products are administered as a single injection or a three-injection regimen. Intra-articular administration is performed by trained clinicians, and ultrasound guidance may be used in difficult joints.

Indications and contraindications: The main indication is symptomatic knee osteoarthritis in adults who have not adequately responded to non-pharmacologic therapy and NSAIDs. They are not effective for inflammatory or crystalline arthritis, and should not be used in the setting of active joint infection. Contraindications include allergy to avian proteins (for avian-derived products), hypersensitivity to hyaluronate, or local infection at the injection site. Caution is advised in patients with bleeding disorders or those on anticoagulants; data on use during pregnancy are limited.

Effectiveness, safety, and regulation: Meta-analyses have reported small-to-moderate improvements in pain and function for some patients, with relief typically lasting several months. Repeated courses may be considered, depending on response and cost considerations. Side effects are usually mild and localized (pain, swelling, warmth at the injection site); serious adverse events are rare. Regulatory status varies by country, with multiple brands approved for knee osteoarthritis in many regions; guidelines on routine use differ, reflecting heterogeneous evidence. Viscosupplementation is not a cure and is part of a comprehensive treatment plan including physical therapy and weight management.
