Svelgeproblem is a term used in Norwegian to describe difficulties with swallowing, a clinical symptom that can affect the transfer of food or liquid from the mouth through the esophagus to the stomach. It can involve problems with the initial swallow (oropharyngeal dysphagia) or with passage through the esophagus (esophageal dysphagia). The sensation may include food sticking, coughing, or a sense of obstruction, and it can occur with solids, liquids, or both.

Causes of svelgeproblem are varied. They include neurological conditions such as stroke, Parkinson’s disease, and multiple sclerosis; muscular or structural disorders affecting the throat or esophagus; mechanical obstructions like tumors, strictures, webs, or rings; motility disorders such as achalasia or diffuse esophageal spasm; and lifestyle or age-related changes, including gastroesophageal reflux disease and reduced saliva production. In some cases, medications or dental issues contribute to swallowing difficulties.

Common symptoms accompanying svelgeproblem include sensation of food sticking in the chest or throat, coughing or choking during meals, regurgitation, unintended weight loss, and sometimes recurrent pneumonia from aspiration. The presentation may be intermittent or progressive, depending on the underlying cause.

Diagnosis typically relies on a medical history and physical examination, followed by targeted tests such as upper endoscopy, a barium swallow study, esophageal manometry, or reflux testing. Patient-reported measures, like swallowing symptom questionnaires, may aid assessment.

Management depends on the underlying cause and may involve diet modification (soft or pureed foods, thickened liquids), swallow therapy with a speech-language pathologist, medications for reflux or motility, and procedures to address obstructions or repair neuromuscular disorders. Early evaluation is advised if there are red flags such as rapidly worsening symptoms, significant weight loss, or persistent coughing with meals.
