The supraspinous ligament is a strong fibrous band that runs along the posterior aspect of the vertebral column in the neck and back. It is located superficial to the interspinous ligaments and extends from the external occipital protuberance of the skull down to the sacrum. Its primary function is to limit excessive flexion of the spine. The ligament is formed by a series of closely packed fibers that are thicker and broader in the thoracic and lumbar regions compared to the cervical region. It plays a crucial role in maintaining spinal stability and preventing excessive movement between the vertebrae. The supraspinous ligament blends superiorly with the nuchal ligament, which continues the fibrous band up to the skull. While generally robust, the supraspinous ligament can be injured, often occurring in conjunction with other spinal injuries, particularly during forceful flexion or hyperextension events. Tears or sprains of this ligament can lead to pain and instability.
