A spherocyte is a type of red blood cell characterized by its spherical shape, deviating from the typical biconcave disk morphology. These abnormal cells are smaller, denser, and lack the central pallor seen in normal erythrocytes, giving them a more uniformly round appearance under microscopy. The formation of spherocytes results from defects in the red blood cell membrane or its associated proteins, which lead to a loss of the cell’s standard biconcave shape.

Spherocytes are commonly associated with hereditary spherocytosis, an inherited hemolytic anemia caused by mutations affecting proteins such as spectrin, ankyrin, or band 3 that stabilize the erythrocyte membrane. In this condition, the compromised membrane integrity causes the red cells to become spherical and more prone to destruction in the spleen. Spherocytes can also occur in autoimmune hemolytic anemia, where antibodies target red blood cell surface antigens, leading to altered cell morphology and increased hemolysis.

Clinically, the presence of spherocytes is often detected through blood smear examination and is indicative of hemolytic processes. Laboratory tests typically reveal increased mean corpuscular hemoglobin concentration (MCHC) and reduced red blood cell deformability. The increased fragility of spherocytes makes them more susceptible to rupture, contributing to anemia symptoms such as fatigue, pallor, and jaundice.

Diagnosis often involves a combination of blood smears, osmotic fragility tests, and genetic analysis. Treatment strategies may include splenectomy to reduce hemolysis in hereditary cases, along with supportive therapies like transfusions and folic acid supplementation. Understanding spherocytes provides insight into conditions affecting red blood cell morphology and hemolytic anemia management.
