Retroduodenal is an anatomical term describing something situated behind the duodenum. It is used in anatomy, radiology, and surgery to denote a posterior relationship to the duodenum and is often applied to describe structures, spaces, or pathologies in the retroperitoneal region.

Anatomical context and relations: The duodenum is primarily a retroperitoneal segment of the small intestine. Behind it lie retroperitoneal structures and tissues, including portions of the pancreas head and major abdominal vessels, as well as the posterior abdominal wall. The retroduodenal region refers to the plane or space behind the duodenum, where disease processes can involve multiple adjacent structures. This position affects both the spread of pathology and the approach to surgical or radiologic management.

Clinical relevance: The retroduodenal concept is important in recognizing conditions that originate behind the duodenum or extend into that area. Posterior duodenal ulcers, for example, can penetrate into adjacent structures such as the pancreas or the gastroduodenal artery, with significant clinical consequences. Retroduodenal hematomas can occur after blunt abdominal trauma or pancreatobiliary inflammation. Tumors, abscesses, and other mass lesions may arise in or extend to the retroduodenal region, potentially causing back or abdominal pain and, in some cases, obstruction.

Imaging and diagnosis: Computed tomography and magnetic resonance imaging are commonly used to evaluate the retroduodenal region and its pathology, with ultrasound offering supplementary information in certain settings. Endoscopic techniques may assist when lesions lie adjacent to the duodenal lumen.
