Rasitusvaiheet, or stages of pressure ulcers (also known as bedsores), describe the progression of tissue damage caused by prolonged pressure on the skin and underlying tissues. These stages help healthcare professionals assess severity, plan treatment, and monitor healing.

The first stage of rasitusvaiheet involves skin discoloration, usually redness in individuals with light skin, or a distinct change in skin color in darker skin tones. The skin remains intact but may be painful, firm, soft, or warm. At this stage, skin damage is superficial and reversible with appropriate care.

The second stage signifies partial-thickness skin loss involving the epidermis and possibly the dermis. The affected area may present as a shallow ulcer, blister, or abrasion, often accompanied by pain or discomfort. The skin may appear red or pink and might be moist.

Stage three includes full-thickness skin loss, extending into the subcutaneous tissue but not reaching underlying muscles or bones. The ulcer appears as a deep crater or cavity, with tissue damage visible. Pus or necrotic tissue may be present.

Stage four involves full-thickness tissue loss with exposure of underlying structures such as muscle, tendons, or bones. The ulcer is extensive, often with dead tissue (necrosis), and may include tunneling or undermining areas. This stage carries a high risk of infection.

Additionally, a fifth category, called unstageable pressure ulcer, indicates the extent of tissue damage cannot be accurately assessed due to the presence of eschar or necrotic tissue covering the wound.

Understanding rasitusvaiheet is vital for appropriate intervention and for preventing the progression of pressure ulcers. Treatment varies depending on the stage, focusing on relieving pressure, wound care, infection control, and promoting tissue repair.
