Postpolycythemia is not a widely used formal medical diagnosis. In most medical literature the term is not defined as a distinct disease entity. When it appears, it is typically as a descriptive label for the period after treatment of polycythemia or for residual or recurrent polycythemia following an intervention. Clinicians usually address the underlying condition—polycythemia vera, secondary polycythemia, or relative polycythemia—rather than the label postpolycythemia itself.

Polycythemia refers to an increased red blood cell mass and can be categorized as polycythemia vera (a myeloproliferative neoplasm often with a JAK2 mutation), secondary polycythemia (driven by hypoxia or other stimuli that increase erythropoietin), or relative polycythemia (hemoconcentration from reduced plasma volume). The term postpolycythemia, when used, may describe the transitional phase after therapeutic measures such as phlebotomy or cytoreductive therapy, during which hemoglobin and hematocrit trend toward target levels.

Clinical implications include monitoring for fluctuations in red cell indices and potential iron deficiency after phlebotomy, which can cause fatigue and other symptoms. Diagnosis hinges on evaluating the underlying polycythemia rather than applying a separate postpolycythemia criterion; this includes testing for JAK2 mutations, assessing oxygen saturation, erythropoietin levels, and iron studies to distinguish ongoing polycythemia from resolution.

Management focuses on treating the underlying cause, maintaining safe hematocrit targets, and addressing complications such as iron deficiency. Regular follow-up and repeat laboratory assessment guide ongoing therapy and prognosis, which vary with the etiology and response to treatment.
