Osteolysis refers to the breakdown or dissolution of bone tissue. This process is a normal physiological event, essential for bone remodeling, a continuous cycle of bone resorption and formation that maintains bone health and strength. However, osteolysis can also occur pathologically, indicating an imbalance where bone resorption outpaces bone formation.

Several conditions can lead to abnormal osteolysis. Tumors, both primary bone cancers and metastatic cancers that have spread to the bone, are a common cause. These tumors can secrete substances that stimulate osteoclasts, the cells responsible for bone resorption, leading to significant bone loss. Inflammatory conditions, such as rheumatoid arthritis and periodontitis, can also trigger osteolysis by releasing inflammatory mediators that activate osteoclasts. Infections in the bone, known as osteomyelitis, can also cause localized bone destruction.

Certain genetic disorders and metabolic conditions can also contribute to osteolysis. For instance, hyperparathyroidism, a condition characterized by overactive parathyroid glands, leads to elevated parathyroid hormone levels, which can increase bone resorption. Additionally, prolonged immobilization or significant mechanical stress on a bone can also induce localized osteolysis.

The clinical presentation of osteolysis varies depending on its cause and extent. Symptoms may include bone pain, swelling, tenderness, and pathological fractures, where the bone breaks with minimal or no trauma. Diagnosis typically involves imaging techniques such as X-rays, CT scans, and MRI, which can visualize the extent of bone loss. Bone scans and biopsies may also be used for further evaluation. Treatment strategies are aimed at addressing the underlying cause of the osteolysis, managing pain, and preventing fractures.
