Neurowaskularymi is a proposed neurovascular syndrome defined by the co-occurrence of neurological symptoms and detectable vascular abnormalities within the brain and related structures. The term fuses concepts from neurology and vascular medicine and appears in a subset of contemporary publications to describe a spectrum of overlapping neural and vascular phenomena.

Clinical presentation is heterogeneous. Patients may experience headaches, transient focal neurological deficits, seizures, or cognitive changes, often with imaging-detected vascular irregularities such as arterial stenosis, dilations, or abnormal vessel reactivity. Onset can occur at various ages and may be episodic or progressive. Etiology remains unclear and may be multifactorial, involving endothelial dysfunction, inflammatory processes, disturbances of cerebral autoregulation, and congenital or acquired vascular malformations.

Diagnosis relies on integrated assessment by neurology and vascular imaging. MRI and MR angiography or CT angiography can identify neural lesions and vascular abnormalities; duplex ultrasound, perfusion studies, and laboratory testing may help exclude mimics such as primary vasculitis or moyamoya disease. There are no universal diagnostic criteria, and classifications are provisional, often dividing cases into predominantly neurogenic, predominantly vasogenic, or mixed phenotypes.

Management emphasizes multidisciplinary care and risk-factor modification. Treatment is tailored to the dominant phenotype and underlying lesions, including antiplatelet therapy when ischemic risk is elevated, management of blood pressure and lipids, and consideration of endovascular or surgical procedures for correctable vascular abnormalities. Prognosis is variable and depends on the nature of the vascular pathology and response to therapy.

Research on neurowaskularymi is limited and ongoing. The term serves as a conceptual framework for describing cases where neurological and vascular features intersect, but standardized criteria and guidelines have yet to be established.
