Neurosteroidogenesis is the process by which neurosteroids are synthesized directly within the brain and nervous system. Unlike steroids produced in peripheral endocrine glands like the adrenal cortex or gonads, neurosteroids are made locally by neurons and glial cells. This localized production allows for rapid and specific modulation of neuronal function without the need for systemic circulation.

The key enzymes involved in neurosteroidogenesis are present in various brain regions. These enzymes convert precursor molecules, often derived from circulating cholesterol or lipoproteins, into active neurosteroids. Examples of neurosteroids include allopregnanolone, pregnenolone, dehydroepiandrosterone (DHEA), and their sulfate conjugates. These molecules are structurally similar to steroid hormones but exert their effects primarily within the central nervous system.

Neurosteroids are known to influence a wide range of neurological processes. They can act as positive or negative allosteric modulators of neurotransmitter receptors, particularly the gamma-aminobutyric acid type A (GABA-A) receptor, affecting neuronal excitability and synaptic transmission. They also interact with other receptors, including NMDA receptors and estrogen receptors, and can influence neurotrophic factors, neuroinflammation, and myelination.

Dysregulation of neurosteroidogenesis has been implicated in various neurological and psychiatric disorders, such as depression, anxiety, epilepsy, Alzheimer's disease, and schizophrenia.
