They can be manufactured for legitimate medical use under strict control or produced illicitly and sold on the street.

Pharmacology and potency vary among fentanyllike substances. Most are highly lipophilic, allowing rapid entry into the brain and a swift onset of analgesia, euphoria, and potentially dangerous respiratory depression. While some analogs have potency close to fentanyl, others are substantially more potent. Carfentanil, for example, is among the most potent fentanyl-related compounds and carries a markedly higher overdose risk. Because of this variability, dosing and risk profiles differ across substances and preparations.

Fentanyllike compounds appear in illicit drug markets disguised as or mixed with other drugs. They have been found in counterfeit pills, powders, and liquids, and may be added to heroin, cocaine, or methamphetamine. The presence of extremely potent analogs such as carfentanil has contributed to significant overdose outbreaks in various regions. Public health agencies emphasize the difficulty of relying on standard drug tests to detect all analogs.

Health risks are severe and include life-threatening respiratory depression, coma, and death, particularly with inadvertent exposure or polydrug use. Naloxone is used to reverse overdoses, but higher or multiple doses may be required for some fentanyllike overdoses. Regulation typically classifies many fentanyllike substances as controlled drugs, with scheduling and monitoring aimed at curbing illicit production and distribution. Detection often requires specialized laboratory testing due to evolving analogs.
