Exame observação is a medical assessment technique in which a clinician observes a patient’s behavior, physiological responses, and interactions over a defined period rather than relying solely on direct questioning or static measurements. The method is often employed in fields such as psychiatry, neurology, and developmental pediatrics, where much of the diagnostic information is conveyed through subtle changes in posture, speech, eye contact, or social engagement. In practice, the examiner may use standardized observation protocols, checklists of observable signs, or video recording to capture repeated episodes that might be missed in brief consultations. The goal is to triangulate information from multiple modalities—clinical interview, physical exam, and observation—to construct a comprehensive picture of the patient’s condition.

In Brazil, the term exame observação is also used outside the clinical arena, particularly within educational and vocational settings. Employers and employers’ associations may conduct observational exams (exame de observação de trabalho) to evaluate a candidate’s suitability for a specific task or workplace environment. Similarly, some high schools and universities incorporate observational exams in their admission processes, assessing candidates’ conduct and practical skills through live demonstrations. These non‑clinical observations adhere to strict ethical guidelines regarding privacy and consent, and results are typically combined with written examinations to form a holistic assessment.

While observational exams can enhance diagnostic accuracy and provide richer contextual data, they also present challenges. The subjective nature of observation may introduce bias, and the presence of an examiner can alter the patient’s or candidate’s behavior—a phenomenon known as the Hawthorne effect. To mitigate these issues, many practitioners now employ structured observation tools and, when possible, blind observers. Advances in digital technology, such as wearable sensors and automated video analysis, are also being explored to provide objective metrics that complement human observation.
