Derisest is a term used in dermatology and tissue engineering to denote the deepest functional subregion of the dermis, roughly the lower reticular dermis adjacent to the dermal–subcutaneous junction. The word blends dermis with the superlative suffix -est to indicate a regional emphasis within the dermal layer. It is not universally standardized and is most commonly found in theoretical discussions and in descriptions of engineered skin substitutes that aim to replicate the full depth of dermal tissue.

Anatomically, the dermisest is characterized by a dense extracellular matrix rich in type I and III collagen, alongside elastin fibers and a relatively high density of cross-links. It contains fibroblasts, perivascular cells, and immune cells, with a vascular network that supports deeper dermal perfusion and nutrient exchange. The region provides mechanical strength and stability to the skin, supports adnexal structures such as hair follicles and glands, and participates in wound healing through fibroblast-mediated remodeling and angiogenesis.

In research and medical practice, dermisest-aware models guide the design of dermal substitutes and grafts, aiming to reproduce the mechanical and biochemical milieu of the deepest dermal zone to improve integration with surrounding tissue. Variability exists across body sites, ages, and species, influencing thickness, fiber orientation, and cellular composition. While not universally adopted, the concept highlights the importance of depth-specific properties in both normal skin function and regenerative strategies.
