Carcinogenesis is the process by which normal cells are transformed into cancer cells. This transformation is a multi-step phenomenon that involves genetic and epigenetic alterations. These alterations can accumulate over time, leading to uncontrolled cell growth and division, which are hallmarks of cancer.

The initial steps in carcinogenesis often involve damage to the DNA of a cell. This damage can be caused by various carcinogens, which are agents that promote cancer formation. Carcinogens can be physical, such as ultraviolet radiation from the sun or ionizing radiation, or chemical, such as components of tobacco smoke or certain industrial pollutants. Biological carcinogens also exist, including certain viruses like human papillomavirus (HPV) and hepatitis B virus, and bacteria like Helicobacter pylori.

Once DNA damage occurs, the cell's repair mechanisms may fail to correct the alterations. If these alterations affect genes that control cell growth and division, known as proto-oncogenes and tumor suppressor genes, they can lead to abnormal cellular behavior. Proto-oncogenes, when mutated, can become oncogenes, driving excessive cell proliferation. Tumor suppressor genes, when inactivated, lose their ability to restrain cell growth and promote cell death, allowing damaged cells to survive and multiply.

Further accumulation of mutations and epigenetic changes can lead to the development of a tumor, which may then acquire the ability to invade surrounding tissues and spread to distant parts of the body, a process called metastasis. This entire cascade, from initial damage to invasive cancer, is collectively termed carcinogenesis. The complexity of this process explains why cancer development often takes many years and can be influenced by a combination of genetic predisposition and environmental exposures.
