Calcaneofibular ligament, sometimes spelled calcaneofibular or calcaneo-fibular ligament (CFL), is one of the lateral collateral ligaments of the ankle. It extends from the distal fibula, just inferior to the lateral malleolus, to the lateral surface of the calcaneus near the calcaneocuboid joint. The ligament runs obliquely inferoposteriorly and lies deep to the peroneal tendons as it crosses the lateral ankle.

Function: The CFL helps stabilize the ankle against inversion forces and contributes to restraining posterior translation of the talus. It works in concert with the anterior talofibular ligament and posterior talofibular ligament as part of the lateral ligament complex. The CFL is relatively taut in neutral to dorsiflexion and becomes slack with plantarflexion, explaining why some injuries involve different ligaments depending on foot position at injury.

Clinical relevance: The CFL is commonly involved in inversion ankle sprains, either alone or with other lateral ligaments. Isolated CFL tears are less common than ATFL injuries but can occur and may lead to chronic lateral ankle instability if not treated properly.

Diagnosis and treatment: Diagnosis is clinical and supported by imaging when needed (MRI or ultrasound). Treatment is typically conservative for partial tears: rest, ice, compression, elevation, early mobilization, bracing or a boot, followed by physical therapy. Severe or chronic CFL injuries, or recurrent instability, may require surgical repair or reconstruction.
