Antitrombotisk (antithrombotic) refers to agents that reduce the formation of thrombosis by interfering with components of hemostasis. These drugs are used to prevent clot formation in patients at risk of arterial or venous thrombosis and to treat existing clots in conjunction with other therapies.

The main categories are anticoagulants, antiplatelet agents, and thrombolytics. Anticoagulants interfere with the coagulation cascade, examples being vitamin K antagonists such as warfarin; direct oral anticoagulants such as dabigatran, rivaroxaban, apixaban, and edoxaban; and parenteral heparins. Antiplatelet drugs inhibit platelet activation or aggregation, including aspirin and P2Y12 inhibitors such as clopidogrel, ticagrelor, and others. Thrombolytics promote fibrinolysis to dissolve existing clots, with agents such as alteplase.

Indications include prevention of venous thromboembolism after surgery, treatment of deep vein thrombosis and pulmonary embolism, stroke prevention in atrial fibrillation, management of acute coronary syndromes, and prevention of thromboembolism in patients with mechanical heart valves or after coronary interventions. The choice and combination of therapies depend on the site and type of thrombosis, patient risk factors, and comorbidities.

Monitoring and administration vary by agent. Warfarin requires regular INR monitoring; DOACs require dosing adjustments for renal function and generally do not require routine coagulation testing. Antiplatelets are typically prescribed for cardiovascular risk reduction, with bleeding risk management. Thrombolytics are used in acute settings and carry substantial bleeding risk.

Risks include bleeding, including intracranial hemorrhage, drug interactions, and, for certain agents, specific adverse effects such as heparin-induced thrombocytopenia. Reversal strategies exist for many drugs (for example vitamin K for warfarin, protamine for heparin, and specific antidotes for DOACs). The choice of antitrombotisk therapy depends on individual risk assessments and clinical guidelines.
