UWløsning, usually referred to as the University of Wisconsin solution, is a preservation solution used to store donor organs during cold ischemia before transplantation. It is designed for static cold storage at temperatures near 0 to 4°C and is commonly employed for kidneys and livers, with use in pancreas and, to varying extents, other solid organs depending on center protocols. It is one of the widely used solutions in organ preservation, often marketed under the brand name Viaspan.

Development and context: The solution was developed at the University of Wisconsin in the 1980s and gained widespread adoption in the following decade, helping to improve outcomes compared with earlier preservation methods. It remains a standard option alongside other preservation strategies and solutions such as HTK and Collins solution, each with its own profile of advantages and indications.

Composition and mechanism: UW solution is a buffered crystalloid formulation that contains ions such as potassium and phosphate, along with protective additives. Key components include impermeants like lactobionate and raffinose to limit cellular swelling, a colloid such as hydroxyethyl starch to maintain oncotic pressure, and magnesium to support membrane stability. The combination aims to reduce metabolic activity during storage, preserve cellular energy stores, minimize edema, and lessen ischemia-reperfusion injury when the organ is implanted.

Clinical considerations: Handling requires strict cold-chain management, as preservation times and organ suitability vary by type and protocol. While UW solution is effective for many organs, centers may tailor their approach using alternative solutions or machine perfusion to optimize graft quality and long-term function.
