Samarium-153-EDTMP is a radiopharmaceutical agent that combines the beta-emitting isotope samarium-153 with the chelating ligand ethylenediamine tetramethylene phosphonate (EDTMP). It is used primarily for palliative treatment of bone pain in patients with skeletal metastases from cancers such as prostate and breast.

Mechanism and properties: The EDTMP chelate targets bone by binding to hydroxyapatite, concentrating in regions of increased osteoblastic activity typically associated with metastases. The radiopharmaceutical delivers localized radiation via beta emissions to these sites, providing pain relief. Samarium-153 has a physical half-life of about 46 hours, and it also emits gamma photons that permit imaging and dosimetry to verify distribution.

Administration and dosing: The agent is given by intravenous injection as a sterile solution. The dose is adjusted to the patient’s body weight and clinical condition, with typical regimens around 1 mCi/kg (approximately 37 MBq/kg), though total activities and schedules may vary by protocol and region. Re-treatment may be considered in some cases if hematologic reserves permit.

Clinical use: Sm-153-EDTMP is indicated for palliation of bone pain in patients with skeletal metastases. It can provide symptomatic relief within weeks and is not curative. Imaging of gamma emissions can be used to assess biodistribution and assist in monitoring.

Safety and monitoring: Hematologic toxicity, including transient leukopenia and thrombocytopenia, is the primary safety concern.
