Perfusion-weighted imaging (PWI) is a magnetic resonance imaging (MRI) technique used to assess blood flow in tissues. It is particularly valuable in the diagnosis and management of stroke, as it can help identify the extent of ischemia (reduced blood flow) and the presence of reperfusion (restoration of blood flow). PWI is based on the principle that the signal intensity in MRI images is influenced by the flow of blood. By using specialized pulse sequences, PWI can measure the arrival time of blood to a tissue, the peak of blood flow, and the time to peak flow, providing quantitative information about blood flow dynamics.

There are several types of PWI techniques, including time-of-flight (TOF), phase-contrast (PC), and arterial spin labeling (ASL). TOF PWI uses the inflow of blood to create a signal, making it sensitive to high flow rates. PC PWI measures the velocity and direction of blood flow, providing detailed information about flow dynamics. ASL PWI uses radiofrequency pulses to tag water molecules in the blood and then measures the signal from these tagged molecules as they arrive at the tissue of interest, offering a non-invasive method to quantify blood flow.

PWI is commonly used in clinical settings to assess the efficacy of thrombolytic therapy, evaluate the effectiveness of endovascular interventions, and monitor the progression of ischemic stroke. It can also be used to guide treatment decisions and assess the risk of recurrent stroke. The technique is non-invasive, does not use ionizing radiation, and can be performed repeatedly without cumulative radiation exposure, making it a valuable tool in the management of stroke patients. However, PWI has limitations, including sensitivity to motion artifacts and the need for specialized equipment and expertise.
