Neuromodulation refers to the targeted alteration of nervous system activity through the delivery of stimuli to neural tissue. Unlike simple neural stimulation, neuromodulation aims to modulate neuronal excitability, synaptic plasticity, and network dynamics to achieve therapeutic effects or research insights. Stimuli may be electrical, chemical, magnetic, or optical and can be delivered invasively or non-invasively. The goal is to adjust neural circuits rather than to trigger specific motor outputs directly.

Common non-invasive methods include transcranial magnetic stimulation (TMS), transcranial direct current stimulation (tDCS), and transcutaneous electrical nerve stimulation (TENS); common invasive methods include deep brain stimulation (DBS), spinal cord stimulation (SCS), and vagus nerve stimulation (VNS). In research settings, optogenetics and chemogenetics provide precise control in animal and some human studies. Pharmacological neuromodulation uses implanted pumps or targeted drug delivery to influence neurotransmitter systems.

Clinically, neuromodulation is used for chronic pain, movement disorders (e.g., Parkinson's disease, essential tremor), epilepsy, and treatment-refractory depression or obsessive-compulsive disorder. Other applications include migraine, obesity, and tinnitus, among others. Regulatory status varies: DBS, VNS, and TMS have approved indications in many jurisdictions; non-invasive methods are often used off-label or in experimental contexts.

Risks include infection, bleeding, device malfunction, skin irritation, mood changes, or seizures, depending on modality. Efficacy is highly condition- and patient-specific, and long-term outcomes require careful monitoring. Current research focuses on closed-loop systems that adjust stimulation in real time based on physiological feedback, personalized parameter optimization, and less invasive approaches with improved targeting. Ethical considerations address autonomy, privacy, and the long-term impacts on neural function.
