Chemistry and mechanism: Methenamine is formed from formaldehyde and ammonia. In urine, especially when the urine is acidic, methenamine decomposes to formaldehyde and ammonia. The released formaldehyde is bactericidal within the urinary tract, helping to prevent bacterial growth when urine pH is maintained in the acidic range.

Forms and administration: The active ingredient is typically supplied as methenamine salts, including methenamine hippurate and methenamine mandelate. These salts are administered orally and prescribed for long-term use in individuals with recurrent UTIs. Dosing and the need for urine acidification can vary by product and clinical context; some regimens combine methenamine with an acidifying agent or recommend dietary measures to support urinary acidity.

Uses and limitations: Methenamine-containing preparations are not intended for the treatment of an active UTI but for prophylaxis in selected patients. Their efficacy depends on maintaining an adequately acidic urine (often pH below about 5.5). They may be avoided in people with certain kidney conditions or in situations where urine acidification cannot be reliably achieved.

Safety and interactions: Common adverse effects include gastrointestinal upset and urinary irritation; hypersensitivity reactions are rare. Drugs or conditions that raise urine pH (alkalinize urine) can reduce effectiveness, while mild acidification may enhance activity. Patients should follow medical guidance on duration of use, dosing, and any concomitant medications.
