InotropieManagement refers to the medical practice of selecting and adjusting drugs that increase cardiac contractility and improve cardiac output in patients with impaired perfusion due to heart failure or shock. The aim is to restore hemodynamic stability while minimizing risks and addressing reversible factors such as volume status and afterload.

Indications for inotrope management include acute decompensated heart failure with low output, cardiogenic shock, perioperative support after cardiac surgery, myocarditis, and certain cases of sepsis with myocardial depression. It is typically implemented in intensive care or advanced heart failure settings, used after careful assessment of volume status and underlying causes.

Common agents used in inotrope management include dobutamine, dopamine, milrinone, and levosimendan (where available). Dobutamine is often chosen for low cardiac output with adequate blood pressure; dopamine provides inotropic support with variable vasopressor effects depending on dose; milrinone is a phosphodiesterase inhibitor that increases contractility and causes vasodilation; levosimendan increases calcium sensitivity and may have longer-lasting effects in some protocols. Drug choice depends on blood pressure, systemic vascular resistance, renal function, and arrhythmia risk. In some cases, inotropes are combined with vasopressors, such as norepinephrine, to maintain adequate perfusion while supporting blood pressure.

Monitoring during inotrope management includes continuous hemodynamic assessment, arterial pressure monitoring, and frequent echocardiography to track cardiac function. Lactate levels, urine output, and signs of organ perfusion are used to gauge response. Titration aims for sufficient perfusion with stable heart rate and blood pressure, while avoiding excessive vasodilation or tachyarrhythmias. Weaning or escalation may be necessary based on the patient’s trajectory and underlying condition.
