Dosiseering

Dosiseering, often translated as dosing regimen design, is the planning and specification of how a drug should be administered to achieve the desired therapeutic effect while minimizing harm. It encompasses the amount of drug, the frequency and duration of administration, the route, and any special timing considerations.

A dosing regimen typically includes the dose (how much), the interval (how often), the total duration, the route (oral, intravenous, etc.), and any special features such as loading doses, maintenance dosing, or tapering. The goal is to maintain drug exposure within a target therapeutic range over time.

Dosing decisions rest on pharmacokinetics and pharmacodynamics, including drug half-life, absorption, distribution, metabolism, and excretion, as well as drug interactions and patient factors. Individual considerations include age, body weight, renal and hepatic function, pregnancy status, comorbidities, and concomitant medications.

Clinical approaches to dosiseering include evidence-based guidelines, population pharmacokinetic modeling, and therapeutic drug monitoring when appropriate. These tools help tailor regimens to individuals and adjust for changes in organ function, infection severity, or treatment responses.

Common contexts are antibiotics, anticoagulants, analgesics, and chemotherapies, as well as pediatric dosing where weight-based calculations are used. Accurate dosising reduces adverse effects and resistance, while poor adherence or incorrect adjustments can lead to treatment failure or toxicity.
