This article provides a concise overview of the vestibular system and its principal structures, which are commonly associated with similar-sounding terms.

The vestibular system is part of the inner ear and brain that controls balance, gaze stabilization, and spatial orientation. It includes the vestibule, consisting of the utricle and saccule, and the three semicircular canals (anterior, posterior, and horizontal). The vestibular nerve, a branch of the vestibulocochlear nerve (cranial nerve VIII), transmits sensory information from these structures to the brain.

Within the vestibule, the otolithic organs—the utricle and saccule—contain hair cells that detect linear accelerations and head position relative to gravity, aided by otolith crystals (otoconia). The semicircular canals detect angular head movements through hair cells located in the ampullae of each canal. Movement of fluid within the canals bends the hair cells, generating neural signals.

Signals from the vestibular apparatus are processed by the vestibular nuclei in the brainstem and integrated with visual and proprioceptive inputs to maintain balance and coordinate eye movements. Dysfunction can cause vertigo, disequilibrium, nystagmus, or abnormal responses to movement. Diagnostic approaches include caloric testing, rotary chair testing, vestibular-evoked myogenic potentials (VEMP), and head impulse testing. Treatments range from vestibular rehabilitation therapy to symptom-relief medications, with surgical options in select disorders such as Ménière’s disease.
