Thyroiditis refers to inflammation of the thyroid gland and can be acute, subacute, or chronic. Common types include autoimmune Hashimoto’s thyroiditis, subacute (de Quervain) thyroiditis, painless postpartum or silent thyroiditis, and rarely Riedel’s thyroiditis. The inflammation can cause phases of thyrotoxicosis followed by hypothyroidism, or progress to chronic hypothyroidism due to gland destruction. Etiologies include autoimmune processes, viral infections, and, in some forms, drug- or radiation-related effects.

Clinical features and diagnosis vary by type. Symptoms may include neck pain and tenderness (more typical of subacute thyroiditis), fatigue, weight changes, palpitations, and heat or cold intolerance. Diagnostic workup usually includes serum TSH and free T4, thyroid antibodies (such as anti-TPO and anti-thyroglobulin), and inflammatory markers (ESR/CRP) when appropriate. Thyroid ultrasound can reveal characteristic inflammatory changes, and radionuclide iodine uptake helps distinguish thyroiditis from other causes of thyrotoxicosis.

Management and prognosis depend on the specific type and disease phase. Hyperthyroid symptoms during the early inflammatory phase may be managed with beta-blockers; NSAIDs or corticosteroids can relieve pain and inflammation in subacute thyroiditis. In autoimmune Hashimoto’s, long‑term thyroid hormone replacement may be needed if hypothyroidism persists. Many forms are self-limited, though some may result in permanent hypothyroidism.
