The sacroiliac joints, often referred to as SI joints, are two small joints located on either side of the pelvis, connecting the sacrum (the triangular bone at the base of the spine) to the ilium (the large, flat bones that make up the sides of the pelvis). These joints are characterized by a rough, irregular surface that interlocks, allowing for a limited amount of movement.

The primary function of the SI joints is to bear weight and transmit forces between the upper body and the lower extremities. They play a crucial role in maintaining stability during activities such as walking, standing, and lifting. The joints are supported by a complex network of strong ligaments, which contribute to their stability and limit excessive motion.

Dysfunction or inflammation of the sacroiliac joints, known as sacroiliitis or SI joint dysfunction, can lead to pain in the lower back, buttocks, and sometimes the legs. This pain can be exacerbated by activities that put stress on the joints, such as prolonged standing or sitting, climbing stairs, or transitioning from sitting to standing. Various factors can contribute to SI joint problems, including injury, arthritis, pregnancy (due to hormonal changes that loosen ligaments), and biomechanical issues. Diagnosis often involves a physical examination, imaging tests like X-rays or MRI, and sometimes diagnostic injections. Treatment strategies aim to reduce pain and inflammation and restore proper function, and can include physical therapy, medication, and in some cases, injections or surgery.
