Sacroiliac joints are articulations located where the sacrum, the triangular bone at the base of the spine, meets the iliac bones of the pelvis. There are two such joints, one on each side of the sacrum. These joints are formed by the articulation of the auricular surfaces of the sacrum and ilium, which are covered with cartilage. They are strongly supported by a complex network of ligaments, including the anterior, posterior, and interosseous sacroiliac ligaments.

The primary function of the sacroiliac joints is to transmit weight from the upper body to the lower limbs. They also play a role in shock absorption and provide stability to the pelvis during movement. While they allow for a limited range of motion, primarily subtle movements of rotation and gliding, their mobility is significantly less than that of other synovial joints. The forces they withstand can be substantial, contributing to their susceptibility to dysfunction.

Dysfunction of the sacroiliac joints, often referred to as sacroiliac joint pain or sacroiliitis, can lead to discomfort or pain in the lower back, buttocks, and sometimes down the leg. This pain can arise from various causes, including trauma, repetitive stress, inflammatory conditions like ankylosing spondylitis, or degenerative changes. Diagnosis typically involves a combination of physical examination, imaging, and sometimes diagnostic injections. Treatment approaches vary depending on the underlying cause and may include physical therapy, medication, injections, or in some cases, surgical intervention.
