Rule out is a clinical decision-making term used to describe the process of evaluating a patient to determine whether a particular diagnosis can be excluded. In medical contexts, to rule out a condition means to gather evidence through history, physical examination, and diagnostic testing that renders that condition unlikely enough to dismiss it as a probable cause of the patient’s symptoms. The phrase is not a diagnosis itself; it reflects the clinician’s intent to exclude certain possibilities from the differential diagnosis.

Clinicians use rule-out reasoning as part of triage, initial assessment, and ongoing workup. Tests are chosen for their ability to decrease the probability of disease and are interpreted in the context of pretest probability. A condition may be ruled out when sensitive tests are negative and the remaining clinical likelihood becomes very small, or when the combination of test results and observation argues against the diagnosis. Nevertheless, no test is perfect; false negatives and false positives can occur, and timing matters, so some conditions can only be excluded after careful follow-up or serial testing.

Common examples include ruling out acute coronary syndrome in patients with chest pain, ruling out meningitis in febrile patients with neck stiffness, or ruling out pulmonary embolism in shortness of breath. The term is widely used in emergency departments and hospital medicine, but many clinicians prefer more precise language such as exclude, not suspected, or disease unlikely after testing.
