Rotfyllinger, or root canal fillings, are endodontic procedures designed to treat teeth with infected or compromised dental pulp by cleaning, shaping, disinfecting, and sealing the root canal system. The aim is to remove diseased tissue, prevent reinfection, and preserve the natural tooth.

Indications include irreversible pulpitis, pulp necrosis, or periapical infection resulting from extensive decay or trauma. After root canal treatment, the tooth is typically restored with a crown or other permanent restoration to protect the remaining tooth structure.

The procedure begins with an access cavity to reach the pulp chamber, followed by removal of pulp tissue and microbes. The canals are cleaned, shaped, and irrigated, then obturated with a filling material, most commonly gutta-percha and a sealing paste. Techniques vary, with single- versus multi-visit sessions and methods such as cold lateral condensation or warm vertical condensation. In recent years bioceramic sealers and advanced obturation materials have become common.

Materials include gutta-percha, root canal sealers, and, in some cases, calcium silicate cements. The choice of materials and technique influences the quality of the obturation and long-term seal.

Outcomes depend on thorough cleaning and a durable coronal restoration. When performed appropriately, rotfyllinger have favorable long-term success rates, though risks exist, including reinfection, persistent symptoms, instrument separation, or root fracture, which may require retreatment or surgical intervention.
