The concept emphasizes a coordinated, patient-centered pathway that spans acute care, rehabilitation services, and community support to maximize independence after illness, injury, or disability.

The coinage appears in early 21st-century European discussions of multidisciplinary care models, reflecting a shift from fragmented services to continuous care across settings. In this usage, rehabilitationare encompasses not only therapies aimed at physical recovery but also strategies to support participation in work, education, and social life.

Core elements include early comprehensive assessment; goal-oriented planning; multidisciplinary teams (including physicians, physical and occupational therapists, speech-language pathologists, psychologists, social workers); patient and family involvement; evidence-based interventions; and systematic outcome measurement.
