Postencephalitic is an adjective used to describe conditions that occur after an episode of encephalitis, an inflammation of the brain that can result from infections (viral, bacterial) or autoimmune processes. Postencephalitic conditions may form part of a broader post-infectious syndrome and can involve residual cognitive, behavioral, motor, and sensory impairments.

Common manifestations include cognitive impairment (memory and executive dysfunction), personality and behavioral changes, headaches, fatigue, sleep disturbances, seizures, and various motor problems such as tremor, rigidity, bradykinesia, ataxia, or focal weakness. In some historical contexts, postencephalitic parkinsonism described parkinsonian features following encephalitis lethargica, illustrating how sequelae can resemble other neurological disorders.

Diagnosis relies on the patient’s history of a prior encephalitic episode, followed by the emergence of new neurological or psychiatric symptoms after the acute phase. Neuroimaging (typically MRI), electroencephalography, and cerebrospinal fluid analysis may aid assessment, but findings are variable. It is important to exclude other conditions that could explain the symptoms. Diagnosis often involves a multidisciplinary approach.

Treatment is supportive and symptom-directed, focusing on rehabilitation (physical, occupational, and speech therapy), management of seizures, and treatment of movement disorders or psychiatric symptoms as needed. Immunotherapies may be considered in suspected autoimmune post-infectious processes. The prognosis varies; some individuals experience partial improvement, while others sustain persistent deficits. Regular follow-up and tailored rehabilitation are commonly recommended.
