Neuropatioina is a term used in speculative neurology to describe a proposed syndrome characterized by peripheral neuropathy with autonomic dysfunction and, in some reports, mild central nervous system signs. It is not currently recognized as a distinct clinical entity in major medical classifications or by leading professional bodies.

Clinical features commonly cited include distal sensory symptoms such as numbness, burning, and paresthesias, often in a symmetric pattern. Autonomic involvement may manifest as orthostatic intolerance, abnormal sweating, gastrointestinal motility disturbances, or urinary symptoms. The onset is typically in adulthood with variable progression and may be intermittent.

Most accounts hypothesize a multifactorial pathophysiology, involving immune-mediated damage to small nerve fibers, dorsal root or autonomic ganglia, and possible interactions with metabolic risk factors or prior infections. Because the syndrome is not established in formal nosologies, there is no single validated diagnostic test. Evaluation typically relies on clinical assessment, specialized sensory testing, and, when available, skin punch biopsy showing reduced intraepidermal nerve fiber density, along with autonomic testing. Nerve conduction studies may be normal, especially when small-fiber fibers predominate. Autoimmune panels or paraneoplastic testing are considered in select cases after exclusion of other etiologies.

Management focuses on symptom control and functional support, using standard treatments for neuropathic pain (for example, anticonvulsants or antidepressants) and interventions for autonomic symptoms. Addressing comorbid conditions and engaging multidisciplinary care are common approaches. Given its hypothetical status, research on biomarkers, pathophysiology, and therapeutic targets remains limited.
