Nearparalysis is a descriptive term used to describe a state of severe motor impairment in which voluntary movement is markedly reduced but not completely abolished. It is not a formal diagnostic category in most clinical guidelines, but appears in case reports and reviews to describe transient or partial motor failure.

It is distinct from paresis, which denotes weakness with preserved some movement, and from paralysis, which implies complete loss of voluntary movement. Nearparalysis suggests a broader impairment that significantly limits function while allowing some activity, and it may fluctuate with time or with effort.

Clinical features can involve a single limb or multiple regions. Affected muscles show marked weakness and reduced dexterity, sometimes with fatigue-related fluctuation. Deep tendon reflexes may be diminished or pathologic depending on the underlying lesion. Sensory changes are not required and may be absent. The presentation often depends on the etiology and may be transient or progressive.

Possible causes include vascular events affecting motor pathways, partial nerve compression or radiculopathy, demyelinating or inflammatory processes, metabolic or toxic disturbances, and medication effects. Functional neurological disorders (conversion disorder) can also present as nearparalysis. A comprehensive evaluation is needed to distinguish organic from non-organic causes and to identify any reversible condition.

Evaluation typically includes a detailed neurologic examination, imaging (such as MRI or CT) when appropriate, electrophysiological studies (EMG/NCS), and laboratory tests to assess metabolic or autoimmune etiologies. Management focuses on treating the underlying cause, safety and supportive care, and rehabilitation through physical therapy. Prognosis varies widely, from full recovery to persistent disability, depending on the underlying mechanism. As a term, nearparalysis lacks standardized criteria and is often replaced by more specific diagnostic labels in clinical practice.
