Maximum tolerated dose

Maximum tolerated dose (MTD) is the highest dose of a drug or treatment that can be given to a patient without causing unacceptable adverse effects in a defined population. The concept is central to dose-escalation studies, particularly early-phase (phase I) trials in oncology, where the goal is to identify a dose that balances safety with potential efficacy.

Determination and definition

MTD is determined during dose-escalation cohorts by monitoring for dose-limiting toxicities (DLTs), which are predefined adverse events of a certain severity occurring within a specified observation window. The dose at which the incidence of DLTs reaches a predefined threshold defines whether that level is tolerable. The MTD is usually the highest dose level at which the incidence of DLTs remains below the threshold, with escalation halted when the threshold is exceeded. Common clinical designs include the 3+3 design and more advanced Bayesian or continual reassessment methods. The recommended phase II dose (RP2D) is often derived from the MTD but may consider pharmacokinetic, pharmacodynamic, and tolerability data.

Applications and scope

In cytotoxic chemotherapy, MTD has historically guided dosing because higher doses can improve tumor kill but increase toxicity. In targeted therapies or immunotherapies, the relevance of MTD may be reduced, as therapeutic benefit can occur at lower doses or be limited by distinct toxicity profiles. In many programs, the RP2D is chosen to optimize the balance between safety and potential activity, rather than strictly maximizing dose.

Limitations

MTD does not always reflect the optimal therapeutic window for all patients or tumor types. It can be confounded by cumulative toxicity, interpatient variability, and differences between short-term DLTs and long-term tolerability. Preclinical MTD in animals helps inform starting doses but may not predict human outcomes precisely.
