Iskemiske is a term used in medicine to describe tissues or conditions that suffer from ischemia, a reduction in blood flow that deprives tissue of oxygen and nutrients necessary for normal function. Ischemia can be acute or chronic and may affect various organs, most commonly the heart, brain, intestines and limbs. The underlying causes include occlusion of blood vessels due to atherosclerotic plaque, thrombosis or embolism, vasospasm, or systemic hypoperfusion from severe hypotension or shock.

Pathophysiology centers on a mismatch between blood supply and metabolic demand. When perfusion falls, cells shift to anaerobic metabolism, leading to ATP depletion, acidosis, and disruption of ion pumps. Prolonged ischemia causes cellular injury and can result in irreversible cell death. Reperfusion after a period of ischemia can introduce oxidative stress and inflammatory responses, which may worsen tissue injury in some settings.

Clinical presentation varies by affected organ. Myocardial ischemia may present as chest pain or dyspnea and can progress to infarction. Cerebral ischemia can cause sudden neurological deficits or stroke. Mesenteric ischemia presents with abdominal pain out of proportion to examination findings, while limb ischemia causes pain, pallor and impaired pulses. Diagnostic approaches combine clinical assessment with imaging and biomarkers: electrocardiography and troponin for cardiac ischemia; Doppler ultrasound, CT or MR angiography for vessel patency; perfusion imaging as indicated.

Treatment aims to restore perfusion promptly and address the underlying cause. This may involve pharmacologic vasodilation, antiplatelet or anticoagulant therapy, thrombolysis, or surgical/revascularization procedures, as well as risk-factor modification to prevent future ischemic events. Prognosis depends on the organ involved, the duration of ischemia, and the speed of reperfusion.
