Intakeoutput, often abbreviated I&O, is the measurement and recording of all fluids entering and leaving a patient over a defined period, typically 24 hours. It is used to assess fluid balance, hydration status, and organ function, especially in hospital settings.

Intake includes all ingested fluids such as beverages, soups, and intravenous fluids; it also encompasses nutritional solutions and medications given in fluid form. Output includes urine, vomit, diarrhea, gastric aspirate, wound drainage, and other measurable losses from drains or tubes. Insensible losses from respiration and skin are not measured directly and are sometimes estimated in calculations.

Measurement and documentation: volumes are recorded using standardized charts or electronic records. Urine is measured with a collection container; other outputs are measured or estimated as feasible. Net fluid balance equals total intake minus total output. A positive balance indicates net fluid gain; a negative balance indicates net loss. Clinicians interpret balance alongside clinical status and patient comorbidities.

Clinical use: monitoring fluid status in dehydration, edema, kidney or heart disease, postoperative care, and critical illness. It guides decisions on fluid therapy, diuretics, intravenous access, and nutritional management.

Limitations: data accuracy depends on thorough collection; insensible losses are estimates, and some outputs may be missed. Individual variation and environmental factors can affect balance calculations.
