Hypoventilation is a medical condition characterized by an abnormally slow or shallow breathing rate, leading to an inadequate exchange of oxygen and carbon dioxide in the lungs. This results in a lower-than-normal intake of oxygen and a higher-than-normal retention of carbon dioxide in the bloodstream. The body's respiratory drive, which regulates breathing, is a complex interplay of chemical signals and neurological signals. When this system is impaired, hypoventilation can occur.

Several factors can cause hypoventilation. These include respiratory depressant medications, such as opioids and sedatives, which can suppress the brain's respiratory centers. Neurological conditions affecting the brainstem, where the respiratory control center is located, can also lead to hypoventilation. Examples include stroke, brain injury, and certain neuromuscular diseases like amyotrophic lateral sclerosis (ALS) or myasthenia gravis, which weaken the muscles involved in breathing. Obesity can also contribute to hypoventilation, particularly in conditions like obesity hypoventilation syndrome, where excess weight impedes the mechanics of breathing and may affect the body's response to carbon dioxide levels. Sleep apnea, especially obstructive sleep apnea, can also cause intermittent hypoventilation during sleep.

The symptoms of hypoventilation can vary depending on the severity and underlying cause. Mild cases may present with shortness of breath, particularly on exertion, and fatigue. As carbon dioxide levels rise (hypercapnia), individuals may experience headaches, confusion, drowsiness, and even loss of consciousness. Chronic hypoventilation can lead to pulmonary hypertension and right-sided heart failure (cor pulmonale) due to the sustained high carbon dioxide levels. Diagnosis typically involves measuring arterial blood gases to assess oxygen and carbon dioxide levels, along with a thorough medical history and physical examination to identify the underlying cause. Treatment focuses on addressing the root cause, which may include adjusting medications, using respiratory support devices like non-invasive ventilation, or managing underlying medical conditions.
