Hoolduskorraldus is a term used in Estonian social welfare to refer to the formal process of organizing and coordinating care for a person who needs assistance due to age, illness, disability or other circumstances. It covers arrangements for home-based services, day services, residential care and other forms of support, with the aim of ensuring comprehensive, continuous care that respects the individual’s needs and preferences while promoting as much independence as possible.

The process typically begins with a needs assessment conducted by a social welfare professional. Based on the assessment, a care plan (hoolduskorraldus) is prepared, detailing the required services, service providers, locations, frequency and duration of care, and any necessary equipment or transportation. The plan also identifies funding sources, eligibility criteria, and the responsibilities of the municipality, service providers and the person receiving care (as well as any family members or legal representatives).

Implementation is carried out by contracted service providers or municipal staff, with a care coordinator responsible for monitoring progress and updating the plan as circumstances change. The person receiving care and, if applicable, their legal representative, participate in planning and must give consent to the services. Respect for privacy, safety and dignity is a central principle of the process.

The care plan is subject to review and potential modification, typically at least annually or when the person’s situation changes. If needs increase or resources are insufficient, alternatives such as additional services or residential care may be considered.
