Hemolysis refers to the rupture or lysis of red blood cells. This process releases hemoglobin into the plasma. Hemolysis can occur intravascularly, meaning within blood vessels, or extravascularly, typically in the spleen or liver.

Several factors can cause hemolysis. These include mechanical trauma, such as from artificial heart valves or microangiopathic hemolytic anemias, where red blood cells are damaged passing through narrow or damaged blood vessels. Certain infections, like malaria, can directly invade and destroy red blood cells. Toxins and venoms from some organisms can also induce hemolysis. Autoimmune conditions, where the body's immune system mistakenly attacks its own red blood cells, are another significant cause. Inherited conditions like sickle cell anemia and thalassemia also lead to premature red blood cell destruction. Even certain medications can trigger a hemolytic reaction.

The consequences of hemolysis depend on its severity and cause. Mild hemolysis may be asymptomatic or cause only slight anemia. More severe or chronic hemolysis can lead to significant anemia, jaundice (due to the buildup of bilirubin, a byproduct of hemoglobin breakdown), and an enlarged spleen (splenomegaly) as it works harder to remove damaged red blood cells. The release of free hemoglobin can also have toxic effects on organs like the kidneys. Diagnosis often involves blood tests to detect anemia, elevated bilirubin levels, and specific markers of red blood cell breakdown. Treatment strategies vary widely depending on the underlying cause and may include addressing infections, discontinuing offending medications, or managing autoimmune or inherited disorders.
