Ekstravaskulær is a term used to describe anything situated outside the vascular system, i.e., outside the blood vessels. It is applied in anatomy, physiology, and pharmacology to refer to compartments, processes, or routes that are not within the lumens of arteries, veins, or capillaries. The word derives from Latin ex- “out of” and vascularis “of a vessel,” and is used in Swedish, Norwegian, and other medical terminology.

In anatomical context, the extravascular space includes the interstitial tissue around vessels and the lymphatic system. Fluids or cells located there are outside the vessel lumens but within the surrounding tissues.

In pharmacology, extravascular administration describes routes that deliver drugs without direct intravascular entry. Examples include oral, subcutaneous, intramuscular, transdermal, sublingual, and inhalation routes. Drugs given extravascularly must be absorbed into the systemic circulation, and their bioavailability depends on the route and the drug’s properties.

Clinical considerations include extravasation, the unintended leakage of intravenous fluids or drugs into surrounding tissue. This can cause local irritation, swelling, and potential tissue injury, especially with vesicant or irritant substances. Prevention focuses on proper IV placement and monitoring, while management involves assessment and appropriate intervention if leakage occurs.
