Décongestive, or décongestionnant in French medical terminology, describes substances and therapies that relieve nasal or sinus congestion. In medical usage, a decongestant reduces mucosal edema and nasal blockage, typically by vasoconstriction or by anti-inflammatory action. They are used to alleviate symptoms of rhinitis, colds, sinusitis, and allergic reactions.

Common forms include topical nasal decongestants such as oxymetazoline and xylometazoline; oral decongestants like pseudoephedrine and phenylephrine. Intranasal corticosteroids (fluticasone, mometasone) are anti-inflammatory agents that help with chronic congestion from allergies; although not traditional vasoconstrictors, they relieve swelling over time with regular use. Some regimens also combine an antihistamine with a decongestant for broader relief.

Mechanism and use: Vasoconstrictors contract nasal blood vessels to shrink swollen mucosa, providing rapid relief. Oral agents reduce systemic vascular tone, contributing to decongestion but with potential systemic effects. Topical decongestants act quickly but are generally advised for short-term use, typically no more than a few days, to avoid rebound congestion (rhinitis medicamentosa). Intranasal corticosteroids take longer to exert full effect but are useful for ongoing allergic congestion and do not cause rebound in the same way.

Safety and considerations: Side effects include increased blood pressure, tachycardia, insomnia, and nervousness; nasal sprays can cause local irritation. Contraindications include uncontrolled hypertension, ischemic heart disease, certain arrhythmias, glaucoma, and interactions with MAO inhibitors for certain oral agents.
