Its usefulness is tempered by limited activity against some organisms (for example, Pseudomonas aeruginosa) and by the potential for resistance through mechanisms such as mecA-mediated methicillin resistance and AmpC beta-lactamase production.

In clinical microbiology, cefoxitin is widely used for susceptibility testing. A prominent example is the cefoxitin disk diffusion test, which serves as a practical surrogate for detecting mecA-mediated methicillin resistance in Staphylococcus aureus. In this test, a zone diameter of 21 millimeters or smaller indicates methicillin resistance (MRSA), while larger zones suggest methicillin-susceptible S. aureus (MSSA). Cefoxitin resistance can also flag AmpC beta-lactamase production in Enterobacterales; there are additional cefoxitin-based or cefoxitin-involved tests (such as cefoxitin with inhibitors) to help distinguish AmpC producers from other resistance mechanisms.

In therapeutic contexts, cefoxitin-based regimens refer to treatment plans that rely on cefoxitin for infections caused by susceptible organisms. It is used for certain intra-abdominal and gynecologic infections among other indications, with dosing and duration varying by infection type and patient factors. Side effects align with other beta-lactams and may include hypersensitivity reactions, gastrointestinal disturbances, and, rarely, Clostridioides difficile infection. Caution is advised in patients with known severe penicillin allergies or in settings where AmpC- or ESBL-producing organisms are common.
