Risk factors for being bruchgefährdet include advanced age, postmenopausal estrogen deficiency, and inherently low bone mineral density. Secondary factors such as chronic gluco­corticoid use, hyperparathyroidism, diabetes, inflammatory diseases, kidney or liver disorders, malnutrition, vitamin D deficiency, smoking, excessive alcohol consumption, sedentary lifestyle, and prior fractures also contribute. Frailty and sarcopenia can compound fracture risk in older adults.

Assessment of fracture risk typically involves bone density testing (DXA) and risk calculation tools such as FRAX, which estimate the 10-year probability of major osteoporotic or hip fractures. Clinical evaluation considers history of fractures, family history, height loss, and functional status.

Prevention and management focus on reducing risk and improving bone strength. Strategies include adequate calcium and vitamin D intake, weight-bearing and resistance exercise, fall prevention, and modification of risk factors (smoking cessation, limiting alcohol, addressing malnutrition). Pharmacological treatments may be used for those at high risk and can include bisphosphonates, denosumab, selective estrogen receptor modulators, teriparatide, or romosozumab, depending on the individual’s profile and comorbidities.
