Atrophia is a term used in medical and anatomical contexts to describe a decrease in the size, weight, or functional capacity of a tissue or organ. In practice, it reflects cell shrinkage and/or a reduction in cell number, which can arise from a variety of causes. The concept is often referred to in English as atrophy, with atrophia serving as the Latin or formal designation in some texts.

Causes and types of atrophia are commonly categorized into physiologic and pathologic processes. Physiologic atrophy occurs as a normal part of aging or development, or after reduced functional demand. Pathologic atrophy results from disuse (for example, immobilized muscles), denervation or nerve injury, ischemia or reduced blood supply, malnutrition, chronic inflammation, or hormonal changes. Endocrine and neurogenic atrophies are notable subtypes, and organ-specific examples include muscular atrophy, cerebral (brain) atrophy, endometrial or glandular atrophy in response to hormonal shifts, and skin or connective tissue thinning with aging.

Mechanistically, atrophia involves limited protein synthesis relative to degradation, increased autophagy, and, in some cases, loss of cells. The histological appearance varies by tissue and cause, ranging from smaller, lighter muscle fibers to loss of neurons or replacement of tissue with fibrous or fatty material. Whether atrophy is reversible depends on the tissue and the underlying cause; disuse and some forms of endocrine atrophy may improve with removal of the cause or rehabilitation, whereas neurogenic or severe ischemic atrophy can be less reversible.

Diagnosis involves clinical assessment, imaging, and sometimes biopsy to determine the cause. Treatment aims to address the underlying cause, supplemented by supportive care such as physical therapy, nutrition, and, where appropriate, hormonal or metabolic management.
